In the motel's bar Jack Reacher overhears a drunken doctor refuse to treat a woman's nosebleed which won't stop. The woman, Eleanor Duncan, is married to Seth Duncan, the scion of the Duncan clan, which holds that part of Nebraska in its iron grip, keeping the population in docile silence. Reacher suspects domestic abuse, and coerces the doctor into doing the right thing.
Jack Reacher is a former United States Army Military Police Major. At age 24, he graduated from the U.S. Military Academy and served 13 years as a Military Policeman, during which time he became part of a military police unit formed to handle tough cases. Though Jack Reacher was demoted from Major to Captain, he regained the rank of Major by the time he mustered out in 1997. He was the recipient of many military awards during his career: the Silver Star, the Defense Superior Service Medal, the Legion of Merit, the Soldier's Medal, the Bronze Star, and a Purple Heart for wounds sustained in the bombing of the US Marine barracks in Beirut in 1983. Since leaving the Army, Reacher has been a drifter.
